Students currently attending Salem Hills High School:
Requests can be made by the student or parent until the student graduates, then it must be made by the student only.
Please visit the Counseling Center and make the request with the secretaries.
Due to privacy laws, you must request your own transcript in writing or in person. There is a $5.00 fee for former students.
Provide the following when requesting a transcript:
1 - Students legal name (the name you used while attending SHHS).
2 - Year your class graduated
3 - Date of Birth
4 - Your signature and phone number
5 - $5.00 fee per copy
6 - Mailing instructions for the return of the transcript. (A transcript is not considered official if it is faxed.)
A mailed transcript request can usually be processed within 24 hours. If you come in to pick up the transcript, it can generally be ready within 15 minutes.
