SHARP Survey Consent Closes 3.24.21

Dear Parents and Students;

Every two years, the state and district conduct a survey regarding student health and risk behaviors.  Information gained from the survey will allow your school district and the State of Utah to continue to provide comprehensive prevention programs for our schools and children.

This year, the State of Utah will be surveying all 10th & 12th graders in their English class.  This survey is optional and anonymous.  Since this survey includes questions regarding risk taking behavior, including drug use, violent behaviors and other personal issues that are not typically discussed in school; parent permission is required for students to participate.  Only students with parent permission will be surveyed.  To ensure that you have received this information, please fill out the permission forms linked below.  If you do not want your student participating in this survey, please indicate so on the permission form.  

Consent survey must be completed by March 24th!!

Sample Questions from the SHARP survey: 

  • “On how many occasions (if any) have you used marijuana in the past 30 days?”
  • “If I had a personal problem I could ask my mom or dad for help?”

  • “When you bought or tried to buy cigarettes in a store during the past 30 days, were you ever asked to show proof of age?”

  • “During the past 30 days, on how many days did you not go to school because you felt you would be unsafe at school or on your way to or from school?
  • “During the past 12 months, how many times were you in a physical fight?”

  • “In an average week when you are at school, on how many days do you go to physical education (PE) classes?
